Description
General Summary/Overview: The LPN is responsible for assisting in the care and treatment of patients, utilizing training skills to ensure the highest quality of care, while maintaining a professional manner at all times.
Essential Functions:
1. Oversee appointment booking and maintain daily schedule for team.
2. Assist with the preparation of equipment and aid physician during treatment, examination, and testing of patient.
3. Oversee tracking system of test results and consults.
4. Coordinate the following: Scheduling all diagnostic tests and admissions; appointments for patients referred to other physicians and then forward all relevant patient information to these physicians. Schedule appointments for physical therapy and dietary consults; completion of referral forms as required. Complete school forms when required.
5. Contact other Health Care Agencies when necessary, i.e. VNA, hospice, Mental Health Care facilities, psychiatric services, Board of Health, etc.
6. Performs the following skills (not inclusive): gives injections (obtain immunizations from appropriate health care facilities); do ear lavages, hearing tests, monitor patients with elevated blood pressures, order literature used for patient education, check and maintain scopes and other equipment, deliver biopsy specimens to pathology labs, forward appropriate data to other medical facilities as necessary.
7. Evaluate and respond to incoming phone calls. Telephone pharmacies with new prescriptions and refills, return other physician’s calls, return all patient calls, notify patients with test results, call acutely ill patients to check progress, document all information relating to patient care in the EPIC system.
8. Travel to alternate offices, as necessary and perform all duties as required by each location.
9. Train new staff in all tasks and monitor team member’s tasks.
10. Attend meetings, workshops and training classes and participates in committees as requested.
11. Maintain professional affiliations and participate in professional development activities.
12. Maintain patient confidentiality.
Preferred Qualifications:
· Education: Formal medical education and training leading to LPN. Certification as an LPN.
- Bilingual Preferred (English/Spanish)
Experience:
· Previous experience in a community based medical practice preferred
· Computer experience required
Knowledge:
· Excellent customer service skills
· Excellent verbal and written communication skills

· Excellent interpersonal skills
Skills/Abilities:
· Ability to prioritize work and respond to changing needs
Physical Demands:
While performing the duties of this job, the employee is frequently required to walk, stand, reach with hands and arms, climb or balance, stoop, kneel, crouch, talk and hear. Requires sitting and standing associated with normal office environment. Manual dexterity for using a computer keyboard and calculator.
Environmental Working Conditions:
Normal office environment. Occasional evening or weekend work.
