Accreditations and Recognitions
Pentucket Medical is committed to striving and maintaining the highest of medical standards and accreditations. Specialty practices such as cardiology, gastroenterology, endoscopy center and diagnostic services have received the highest levels of recognition in their fields.
We are proud of earning Level 3 Recognition from the National Committee for Quality Assurance (NCQA) as a Patient Centered Medical Home Recognized Practice. NCQA is a private, non-profit organization dedicated to improving health care quality. The Patient-Centered Medical Home (PCMH) is a model of care that emphasizes care coordination and communication to transform primary care into “what patients want it to be”.
In 2018, PMA was honored to be named to the 2018 Harvard Pilgrim Health Care Physician Group Honor Roll. Only physician groups with performance in the top 25th percentile of physicians measured nationally on HPHC’s evaluation scale were named to the honor roll. Coordinated Care
Integration of Behavioral Health and Primary Care
The integration of Behavioral Health and Primary Care is the systemic coordination of physical and behavioral health care. This partnership creates a comprehensive and seamless approach to patient centered care for those with multiple health related issues.
Transitional Care
Pentucket Medical’s Transitional Care Team works with patients who are discharged from the hospital and returning home. Coordinating their discharge orders with members of the primary care team helps ensure a patient’s safety and successful recovery.
Elder Wellness
To ensure that our older patients maintain the best of health we require Medicare Welcome and Annual Visits. With the data gathered at these visits, provides primary care clinicians with the information needed to develop an ongoing personalized care plan.
Improving Quality
Telehealth
PMA is investigating the use of telemedicine for all sites. It is currently being used in ExpressCare to help clinicians treat patients with highly specialized health care needs.
Improving patient communications

Data Collection
We measure key quality metrics and compare our performance to national standards to ensure we are achieving the highest quality of care. Health Evaluation and Data Information Set (HEDIS) measures address a broad range of important health issues. Among them are the following:

- Persistence of Beta-Blocker Treatment after a Heart Attack
- Controlling High Blood Pressure
- Comprehensive Diabetes Care
Research
Pentucket Medical is the only healthcare practice in the Merrimack Valley that is actively participating in clinical trials. PMA is committed in improving and offering advanced treatments some of which have received FDA approval in the areas of cardiology and diabetes.
